After the procedure, the pigment may temporarily swell or develop tiny scabs. Through these changes, improvement in freckles can be expected with just one treatment session, and in some cases, improvement is seen within a relatively short period.

For example, for brighter skin tones and shallower pigmentation, the laser is applied more precisely with a smaller spot size and higher energy. Conversely, for skin prone to hyperpigmentation, the output is delicately adjusted to reduce the risk of side effects.

Finely adjusting laser parameters in this way, even when using the same equipment, significantly impacts improving treatment precision and enhancing individual results.

Distinction 5 Treatment plan considering overall pigmented lesions, not just freckles

A significant number of patients who visit for freckle removal also have dermal pigmented lesions such as melasma, in addition to freckles.

While it is possible to focus solely on removing freckles, many patients often wish for improvement in overall mottled pigmentation across the entire face.

Through in-depth consultation with the patient and the use of diagnostic equipment, we accurately classify the pigmented lesions on the face. Subsequently, we carefully discuss and decide on the treatment direction, whether to target only epidermal pigmentation or to aim for overall improvement of pigmented lesions.

What causes freckles?

Freckles are common epidermal pigmented lesions that result from a combination of genetic predisposition and strong UV exposure.

They typically begin to appear in childhood and are more frequently observed in individuals with lighter skin tones.

They primarily occur on areas directly exposed to sunlight, such as the nose and cheekbones, and tend to darken or lighten with seasonal changes.

 

Characteristics of Freckles and Laser Response

Freckles tend to respond relatively well to specific laser wavelengths due to the skin's unique structural and optical characteristics.

One of the wavelengths that can be effective for such lesions is 532nm.

Most freckles are formed by the localized accumulation of melanin granules in the basal layer of the epidermis. Due to this characteristic, the selective action of the 532nm wavelength can be effective for freckle removal.

 

Principle of 532nm Laser Action

The 532nm wavelength is a green-spectrum wavelength with a high absorption rate for melanin pigment. It primarily acts selectively on pigmented lesions located in the epidermal layer of the skin.

Typically, the 532nm laser utilizes the Q-switched method. This method involves instantaneously irradiating high energy output for a short duration, measured in nanoseconds, to selectively destroy only pigment particles.

The irradiated short-pulsed energy is selectively absorbed by melanin and converted into thermal energy. Simultaneously, it creates a photomechanical effect, effectively breaking down melanin particles.

These finely broken-down pigment particles are partly formed into tiny scabs that naturally fall off, while the remaining part undergoes a process of being expelled into the lymphatic system through macrophages in the body.

As pigment particles are removed, the freckle lesions can visibly lighten, and overall improvement can be expected.
