Step 1 – Superficial Adhesion Release

What is Subcision?
Depressed scars exhibit a characteristic where tissue pulls the skin downwards from within.
While the skin surface appears indented from the outside, beneath the dermis,
tissues called fibrous tethers are pulling the scar area downwards.
Only by releasing these adhered tethers can the skin return to its original position.
The procedure that severs these fibrous tissue adhesions is precisely Subcision.
Principle of Subcision Treatment
1. Adhesion Release (Release)
The fibrous tethers deeply embedded in the skin are
delicately separated using a specialized needle or cannula.
Through this process, the adhesive structures pulling the skin downwards are released, and as a result, the depressed scar area can be expected to gradually fill in.
2. Inducing Micro-bleeding (Regeneration Signal)
The micro-bleeding that occurs during the process of separating the adhered tissue
can serve as an important signal to promote new collagen production.
This can induce fibroblast activation, creating an environment where new skin tissue can regenerate around the scar.
3. Promoting Blood Supply and Tissue Regeneration

Our bodies tend to restore damaged areas to a height and texture similar to the surrounding skin when healing.
However, if the scar borders drop abruptly, this natural regeneration mechanism may struggle to work effectively.
Therefore, a process is needed to delicately refine the scar's borders into a gentle slope (shoulderling).
Through this approach, by reducing the height difference with the surrounding skin, when the scar area regenerates, it can be expected to connect more naturally with the adjacent skin, resulting in a less noticeable appearance.
In reality, many people do not achieve satisfactory improvement with traditional Subcision treatment alone, and this is closely related to the issue of scar borders.
Even if the adhesions at the scar base are sufficiently released, if the borders remain like a wall, the regeneration process may have limitations.
Therefore, the shoulderling technique can be seen as an important step that helps to complement the limitations of existing approaches in the treatment of depressed scars with clear borders.
When to consider Multi-Subcision Treatment
